Incident Reporting
& Safety
Challenge is committed to providing a safe and supportive environment. We encourage people receiving support, as well as family members, advocates, and community members, to report any safety concerns or incidents.
Challenge has zero tolerance for abuse, neglect, mistreatment, or exploitation. All staff are trained in abuse prevention, recognizing warning signs, and their responsibility to report concerns. Any suspected abuse or neglect is addressed promptly and in accordance with required reporting and protective procedures.
Jonathan’s Law
Jonathan’s Law requires that certain incidents involving people receiving supports be reported to a designated “Qualified Party” (such as a parent, guardian, or family member).
Challenge will notify the appropriate party as soon as possible, but no later than 24 hours after the incident is discovered. A follow-up summary of actions will be provided within 10 days
Qualified Parties may also request a meeting to discuss the incident, as well as redacted copies of reports or investigation findings, in accordance with applicable requirements and timeframes.
Challenge is committed to meeting all requirements of Jonathan’s Law and ensuring timely, transparent communication with those involved.
