Medicaid Service Coordination


Medicaid Service Coordination, or MSC, has been a thriving program at Challenge since early 2008.  MSC at Challenge strives to move individuals to the highest possible level of independence that they are looking to achieve.  Our experienced coordinators work with individuals, their families and natural supports to gain access to services and supports that not only meet their needs, but also provide a level of service they are fully satisfied with. 

Using a person-centered planning process, individuals will work with their service coordinator to develop, implement and maintain their Individualized Service Plan.  Your service coordinator will be able to provide linkage to a variety of services that can be accessed through the HCBS Waiver as well as service opportunities through various agencies in the community.

To be eligible for services, you must:

  • Have a documented diagnosis of a developmental disability
  • Be in enrolled in Medicaid, or be eligible for Medicaid programs
  • Have a need for ongoing service coordination

Home and Community Based Services (HCBS) is a Waiver program that allows individuals who are eligible for service through the Office for People with Developmental Disabilities to access a variety of programs and supports that promote the level of independence they are interested in achieving.

Melissa Mann, Supervisor of Service Coordination
Mellisa Mann Supervisor of Service CoordinationHolding a degree in Sociology from SUNY Cortland, Melissa has been working in the human service field for several years, with the last three years in Service Coordination.  Melissa has served people in many different human service arenas including children in child care settings, adults working toward their GED in a family literacy program, pregnant and parenting teens, registered child care providers and currently serves children and adults with developmental disabilities.   Melissa finds a great deal of inspiration as a Service Coordinator partnering with individuals and families to advocate for the supports and services needed to work towards their own personal goals.    
When Melissa isn’t providing services at Challenge she can be found at home with her husband, two young daughters and numerous family pets; together they enjoy quality family time, the outdoors and too many extracurricular activities to count. 

Sarah House, Medicaid Service Coordinator
Sarah MSCSarah attended SUNY New Paltz where she earned a degree in Anthropology.  Upon graduating, she entered the field of providing services for individuals with disabilities, gaining experience as a direct support professional.  Sarah celebrated her two year anniversary as an MSC at Challenge in June of 2011.  She finds great fulfillment her role as an MSC, working with people to identify their individual goals, navigating the system of available services, and advocating for the people she works with in support of their decisions.
Sarah is an animal enthusiast, with dogs, cats, fish and frogs as pets.  Sarah feels right at home in Ithaca, with the many local parks and gorges to hike.  She also enjoys spending time in her garden in the warmer months, growing lots of tomatoes.
